Life on Subutex

The Diploma Thesis deals with the Subutex addiction and its effects on the lives of the Subutex users. The main goal of the thesis is to find out how Subutex affects everyday lives of its users from social, health and also other perspectives. There are two main research questions. Firstly, the question is what poses a main problem for the Subutex users in connection with their addiction and secondly, what limitations they face when it comes to their everyday lives. The theoretical part of the thesis includes definition of addiction, describes its phases, causes and other factors connected with it. Further, there is a short overview of currently most commonly used drugs including Subutex. The chapter "Effects of drug addiction" sums up the most common direct effects of drug addiction on social and health wellbeing of the users. The following chapter talks about social work and the importance of case management when dealing with drug addicts, and also the importance of coordination of all phases of rehabilitation. The final pages of the theoretical part mention different ways and kinds of treatments of drug addiction, mainly the substitution treatment. The empirical part of the thesis is focused on research, research questions, methods, and of course, its results. The research was based on semi-structured interviews with 10 interviewees, whose background is also described in more detail. All of the interviewees are clients of "Kontaktní centrum Písek" and all of them are active users of Subutex. The ways of how they get the substance were not considered as a limitation in the research. The results of the research led to several conclusions. Firstly, the main problem the Subutex users have to face is the low living standard as a result of their low economic status and the high price of Subutex. The users also often face comorbidity and their health is rather poor. They most often suffer from hepatitis C gained through needle usage. Further, the interviewees mention negative effects of Subutex usage on their social life and relationships, and also social stigmatisation leading often to social exclusion and worsened mental health. All these aspects negatively impact the quality of life of the Subutex users and hinder their better integration back into society. The thesis as well as its results can be beneficial as a complementary study material or as a source of information for novice workers at social services focusing on drug addiction.

Bio-psycho-social-spiritual model in the buprenorphine substitutional treatment

This thesis is focused on the application of holistic, bio-psycho-social-spiritual model of dependence to concrete therapeutic method, namely to the substitution treatment of persons dependent on opiate addictive substances with synthetic opioid buprenorphine. The aim is to describe the essence of BPSS model of addiction, with the specifics of opiate addiction, and the treatment system based on it. Also, to show substitution therapy as one of the most effective methods that improves quality of a dependent person's life. Further, to confirm the need for a holistic perception of substitution treatment when ersatz drug use itself is not a real chance for dependent persons, unless such treatment is accompanied by psychosocial and spiritual therapeutic approach. Another objective is to map the risk that, in practice, the substitution therapy initiation in a holistic range brings in the context of contemporary reality. Case study of a client included in methadone substitution programme

of bachelor thesis Keywords: methadone- susbstitution therapy- client- substitution programme-causes The bachelor thesis deals with the effect of client participation in the complete substitution treatment programme with methadone, which aims to demonstrate a positive effect not only on the client itself, but also on its vicinity. The theoretical part deals with methadone, its effects and the progressive placing into the treatment of addicted clients, firstly abroad and then in the Czech Republic. It deals with forms of treatment working in the Czech Republic with a detailed description of a complex substitution therapy. At the end of the theoretical part are discussed the causes of origin of an addiction on the addictive substances. The thesis is devoted to the issue of addiction services with a focus on substitution treatment. Theoretical part focuses on presenting of susbstitution treatment, its history and description of the substitution substances and their effects on the human body. Describes the abuse of substitution treatment by problem drug users and tries to map out the various addiction treatment services with a focus on substitution treatment in Prague. The research section defines the attitude of society towards the establishment and functioning of Prague substitution centers. This bacelor work deals with the subjective perception of significance of substitution treatment by it's clients. It's aim is to find out, how the clients of this treatment perceive it's significance primarily in their lives. Out of the data gained from the research it is obvious that despite some negatives of the substitution treatment it's clients considers it to be very positive especially because it allows them to get involve in the society and to live the "normal life". Suprisingly, in comparison with the orientation of intact population the clients almost completely ignore the influence of substitution on their state of health and the risk of transer of infection diseases. Effective factors for improving the quality of life of clients in substitution treatment

Background: Substitution treatment has become a stable part of the addiction services system for drug users. In the beginning, it primarily reacted to healthcare complications related to intravenous substance use and was an alternative for those not able to keep an abstinence. It is proved that only pharmacotherapy is not enough intervention and that it is key to add counseling or therapy as it becomes similarly effective as abstinence-oriented treatment where we work on lifestyle changes in clients. Objective: Identification of effective factors leading to an increase in quality of life from the perspective of clients in substitution treatment in CADAS SANANIM z.ú. Methods: A semi-structured interview and a standardized WHOQOL-BREF questionnaire of the World Health Organization were used to obtain the required data. The descriptive method of thematic (focal) analysis was used to process data from the interviews, and data from the questionnaire was evaluated according to the instructions of the User's Guide of the Czech version of the WHO quality of life. How does the fluid intake affect the gastrointestinal absorbtion of levothyroxine.

Thyroid diseases belong among the most common endocrinopathies, and, therefore, thyroid hormones are among the most commonly prescribed drugs in the Czech Republic, as well as in the world. Even though untreated thyropathies may cause several health complications for the patients, poorly administered treatment can also hurt the health of the individual. Several factors make the treatment difficult. A type of fluid used to ingest the drug belongs among such factors, and it represents the focus of this Master thesis. As is indicated by its title, the thesis focuses explicitly on the gastrointestinal absorption of levothyroxine after being ingested with water, black tea or cow's milk. The thesis was aimed at determining whether and to what extent, if any, the fluids may influence the gastrointestinal absorption of the drug. Based on this premise, research was conducted with six test subjects who underwent a relatively challenging absorption test with four blood samples taken over six hours. The data set (altogether 56 samples taken from all test subjects) had revealed that the gastrointestinal absorption of levothyroxine reached its maximum values when the drug was ingested with water. Mapping the opinions of outpatient addiction tretment facility workers on the possible implementation of substitution treatment for methamphetamine addiction

BACKGROUND: Despite the high number of methamphetamine users in the Czech Republic, the treatment is still only abstinent. Pharmacotherapy in terms of substitution treatment is not yet in place. No study has yet been conducted to examine the views of workers working with methamphetamine users on the possibility of introducing substitution treatment for methamphetamine dependence in the Czech Republic. OBJECTIVE: The aim of the practical part was to map the opinions of outpatient staff on the possibility of introducing substitution treatment for methamphetamine dependence. The monitored area included the evaluation of the success of the treatment of methamphetamine dependence in outpatient settings, disparity in working with users by substance use, percentage of methamphetamine users in the facilities, the suitability of drug addiction to methamphetamine for clients of outpatient facilities and position of substitution treatment for methamphetamine, according to respondents. The origins of harm reduction in the Czech Republic in the context of foreign drug policy

This diploma thesis describes the origins of harm reduction (HR) in the Czech drug policy. It focuses on the role of HR in national strategies and the development of syringe and needle program and substitution treatment. The aim of the thesis is to describe and explain how the HR approach was developed in the Czech environment. Using the framework of policy diffusion, the author explains the mechanisms by which harm reduction was transferred from abroad. Thesis describes the history of drug policy and the development of the harm reduction approach in the world. This information forms the context in which the author advocates HR development in the Czech Republic's drug policy after 1989. By analysing expert texts and expert interviews she explains how the first national strategies were formed in which HR has been included since 1993. Similarly, it deals with the beginnings of syringe exchange, which has been in operation since 1986, and substitution treatment that was first used in 1992 and has been officially operational since 1997. The mechanisms of diffusion of HR from abroad to the Czech Republic are defined. In all three areas, the mechanism of learning is defined.
